What also came to mind for me was what needs to be removed to keep patients from being readmitted? It is important to educate patients, especially elderly patients, that rugs should be removed to avoid tripping. This is more commonly seen when patients are using wheelchairs, walkers, and canes. If oxygen comes into the home it is absolutely critical that the patient is educated on cessation of smoking. Smoking near an oxygen tank can lead to fires. Home health is helpful in many ways. They can assist in medication administration like you said. They can also help in giving baths which may help prevent a fall.
