Article Analysis and Evaluation of Research Ethics

Article Citation and Permalink (APA format)Article 1 Lin, S., Ge, S., He, W., & Zeng, M. (2020). Association between Body Mass Index and Short-Term Clinical Outcomes in Critically Ill Patients with Sepsis: A Real-World Study. BioMed research international2020. URL: https://www.hindawi.com/journals/bmri/2020/5781913/
PointDescription
Broad Topic Area/TitleAssociation between Body Mass Index and Short Term Clinical Outcomes in Critically Ill patients with sepsis
Problem Statement (What is the problem research is addressing?)The problem that was seeing addressed was the effect of being overweight or underweight in clinical outcomes in critically I’ll patients with sepsis.
Purpose Statement (What is the purpose of the study?)To investigate and determine how body mass index affects clinical outcomes in critically ill patients with sepsis.
Research Questions (What questions does the research seek to answer?)How does body mass index affects clinical outcomes in critically ill patients with sepsis?
Define Hypothesis (Or state the correct hypothesis based upon variables used)There is a connection between the BMI of sepsis patients with the mortality rate whereby having BMI index below or above the normal index increases the patients risk of death.
Identify Dependent and Independent Variables and Type of Data for the VariablesThe independent variable is the study exploring the association between BMI and clinical outcomes. The dependent variable is the mortality rate among sepsis patients in relation to their BMI if falling within underweight and overweight range. There both continuous variables like the patients BMI and height and age and categorical variables were like gender either male or female.
Population of Interest for Study8154 is the total population chosen from the Medical Information Centre for Intensive Care
SampleA sample of 7967 patients with sepsis was chosen from the total population of 8154. Those excluded were patients younger than 18, patients with missing weight and height parameters and patients with BMI less than 10 and greater than 60.
Sampling MethodNon probability sampling method because the participants are conveniently available pool of respondents.
Identify Data Collection Identify how data were collectedData was collected through Structure Query Language  used to extract the data in the software Navicat Premium
Summarize Data Collection ApproachData collection was done through already recorded data in the structured query language. The secondary source of data was used. All patients data 18 years or above and diagnosed with sepsis-3 and sequential organ failure (SOFA) score of 2 or more were included. BMI was calculated by weight and height on first day when they were admitted in the ICU. Patients with weights missing  and with BMI less than 10kg/m2 or more than 60kg/m2 The SQL was utilized in data extraction from the Navicat Premium,
Discuss Data Analysis Include what types of statistical tests were used for the variables.  Data were expressed as median (Q1–Q3) or (SD) for continuous variables and as numbers and percentages for categorical variables. The Chi-square test was used for categorical variables and one-way analysis of variance for continuous variables to compare differences in patient characteristics between groups. Cox regression and logistic regression models were used to investigate the independent roles of BMI categories in relation to the primary and secondary outcomes.
Summarize Results of StudyThere was a U shaped relationship between body mass index and 28 day mortality in critically I’ll sepsis patients ,with the lowest 28 day mortality at body mass index range of 30 – 40 kg/ m^2
Summary of Assumptions and Limitations Identify the assumptions and limitations from the article. Report other potential assumptions and limitations of your review not listed by the author.  While the article has not explicitly stated any assumption my review points to some possible assumptions where the researcher assumes that the participants have a sincere interest to participate in the study, have no other motives, and that the inclusion criteria of the sample were appropriate and thus assure that the participants have experienced the same phenomena under study. On the other hand, the authors identify the study limitations as their attempt to exclude false obesity due to interventions by extracting the first BMI after admission in the ICU. Another limitation is that they could definitively determine where the interventions the patients received prior their ICU admission had an impact on their BMI just to mention but a few.

Ethical Considerations

The researchers sought the approval of the Institutional Review Board of the selected project site, namely the Beth Israel Deaconess Medical Center and MIT- affiliated institutions. While patient consent was not required, this resulted from the fact that it was a retrospective study, and the anonymity of the data was secured. In contextual terms, nursing research ethics are derived from that branch of philosophy known as ethics, which specifically address the dynamics involved in decision making of what is right or wrong.AS such scientific research operates within the confines of individual, community, and social values, there is a need to protect the researcher’s dignity in the study and the publication of their information in the research. To the nurse researcher, their ethical considerations have to be informed by three value systems focusing on the nursing profession, society, and science. In nursing culture, ethics are guided by the principles of caring. At the same time, society dwells mainly on human rights in as much as science will emphasize the researcher’s values of scientific inquiry. Subsequently, the ethical issues that predominate this paper, just like in other scientific research papers having human subjects touch on informed consent, the principle of beneficence- not to do harm, respect for confidentiality and anonymity, and respect for personal privacy.

During the sampling stages, informed consent’s ethical issue meant that the participants knowingly, voluntarily, and intelligently freely chose to give their consent clearly and manifestly with duress or coercion. Through informed consent, the patient participating have their autonomy safeguarded all the way through. During the data collection stages, respect for anonymity and confidentiality were safeguarded at this stage which ties closely to the rights of beneficence, respect for dignity, and fidelity. These were safeguarded in that a specific subject could not be linked to their responses since the storage of data collected was anonymized. In instances where the researcher cannot explicitly guarantee anonymity, then confidentiality becomes paramount. To safeguard confidentiality, only the project coordinator knows the confidential details as even the researcher team was not even aware of these details.

Moreover, beneficence means the researcher should not initiate any activity that would directly or indirectly result in the patients’ harm or indirectly due to their study. The same ethical principles were carried through to publish the study results, where no patient identifier was revealed. Most importantly, throughout the research process, the researchers only accessed the participants’ relevant components specific to the study programs.
