The Committee on the Robert Wood Johnson Foundation Initiative (RWJFI) on the Future of Nursing at the Institute of Medicine (IOM) established a goal for a revolutionized healthcare system in its report while acknowledging the needs and limits of the existing system healthcare system. This article examines and explores the impact of the IOM report and the associated state-based action alliances on the current nursing profession, academia, and workforce development and how they promote the profession’s goals.

Robert Wood Foundation Committee Work and IOM Report

The IOM research collaborated with RWJF to formulate critical changes for the future of nursing. According to the National Academy for Sciences, Engineering, and Medicine (2018), the RWJF has developed and implemented actions on nurses’ critical roles regarding the provision of patient care, and the initiation of necessary reforms is concerned. Through the collaboration of these two organizations, different strategies that would enable increased access to healthcare, and reduce healthcare service costs while improving the quality of care were identified. The joint efforts yielded a common ground that helped determine the set objectives, identify barriers, and synthesize sustainable and practical solutions. Notable among these were implementing continuous education, defining the scope of practice of various nurses, data collection, and procedures that significantly improve each of these aspects. Palatnik (2016) posits that RWJF and IOM’s joint efforts led to the action-oriented Future of Nursing Report.

The committee conducted several field visits following every public forum to understand nurses’ plight in academia and health facilities. Equally, the committee collated personal information and public input during the project inquisition. Lastly, the committee carried out literature reviews and wrote research papers on the subject matter.  

Four Key Messages that structure the IOM Report Recommendations

The Committee on the RWJF Initiative on the Future of Nursing at the IOM created a vision whose successful implementation would transform the US healthcare system. The report’s essence is to envision a future healthcare system where quality care is accessible to diverse populations of the US while purposefully promoting wellness, preventing diseases, improving health outcomes, and offering compassion across the lifespan. These aspirations are condensed in the four key messages that are highlighted herein.

Nurses should practice to the full extent of their academic and practical education and training. Suffice it to say that federal and state actions have to update and standardize the scope of practice regulations in ways that capitalize on the optimal capacity and education of APRNs if all Americas are to access needed services in healthcare facilities.

Key Message #2;

Focuses o the fact that nurses should achieve higher education levels and training engrained in an enhanced education system that effectively ensures seamless academic progression. Through this message, the US healthcare system is expected to experience significant changes in its practice environment before and after the nurses in context have received their licenses. For example, nursing interest has grown exponentially, where entry-level baccalaureate programs increased by over 70% between 2005 and 2010(Altman et al., 2016).

In Key Message #3;

Nurses are expected to be full partners with physicians and other healthcare professionals in redesigning the US healthcare system. Under this message being a full partner embraces even the health policy arena. Subsequently, the nurse of the 2020s and beyond must take policy as a concept that they can shape instead of it being something that only happens to the last. At the same time, key messages # 1, 2, and 3 address the need to transform nursing; the fourth message underscores the need for an effective workforce planning and policy that calls for improved data collection and information infrastructure. In summary, the four key messages recommend that nursing has to undergo fundamental transformation within the practice, education, and leadership. 

The practice of nursing has dramatically improved following the four key messages and other influences led by evidence-based practice. Taking interprofessional collaborative teams, nurses have acquired skills in effective communication. Consequently, better patient care has led to better patient outcomes even as the said communication promotes continuous education. Another positive influence of the IOM Report’s key Messages is the changes witnessed in the nursing workforce development. State-based action coalitions work locally at state levels to advocate for healthier communities. Action coalitions work through nurses who are vital partners in offering care and ensuring healthcare equity and quality. The IOM report goals involve better patient experience, improved public health, and affordable healthcare services. Action coalitions involve a diverse team of agencies, policymakers, businesses, and consumers within the state that contribute to sustainable transformations.

The action coalitions seek to advance the IOM report goals and objectives by addressing immediate local and regional concerns. The action coalitions track progress in achieving goals, and share changes, challenges, and implications across the state. The action coalitions identify areas that need improvements within the state, design and share best practices, assess research needs, track progress, and design models for change (National Academies of Sciences, Engineering, and Medicine, 2016).

New York State Action Coalition (2020) posits that adopting novel ideas and improvements would offer better care and promote continuous quality improvement resulting in better patient outcomes and specific individuals’ improved quality of life. Healthcare professionals acknowledge that nurses have a central role to play if patient needs are addressed in their totality.

Like other state action coalitions, the NY Action Coalition emphasizes the need for trust and effective communication among multidisciplinary health care teams. These measures are bound to reduce medical errors and enhance correct diagnosis and timely initiation of appropriate treatment and management.

The action coalitions involve campaigns that advocate for improved healthcare systems in specific states by placing recommendations for changes within the education system, nursing leadership, eliminating practice barriers, supporting collaboration and partnerships, strengthening population diversity, and workforce data.

Lastly, State-based action coalitions were established by launched to help nurses and other medical practitioners achieve the IOM goals. Business partners and nurses lead these coalitions’ drive for changes at every level of governance by charging for healthy communities and healthcare transformation (Campaign for Action, n.d-a). These coalitions, carrier campaign initiatives at the local, regional and national levels.

State Action Coalition in Future of Nursing Essay

NewYork Action Coalition

The NY Action Coalition offers scholarships to deserving nursing professionals to pursue advanced-level education (Campaign for Action, 2020). Additionally, the NY Action Coalition has several initiatives to encourage nurses to pursue higher degrees, like BSN, MSN, DNP, and Ph.D. in nursing. Besides incentivizing trained nurses to advance their education and training, the NY Action Coalition also invests in community education where specific populations and diverse communities are trained to make healthier daily choices.

California Action Coalition

Access to care – California has in place a new law to improve access to care and improve health equity. This law will ensure that 39 million people enjoy improved care access from well-trained and registered nurses. This law requires nurses to practice to the full range of their education and training. The law ensures that consumers get qualified clinicians, eliminates regulations to access healthcare providers, and offers means of reducing paperwork to save on consumer time (Lee, 2020). Transforming nursing leadership – California action coalition also advocates for transforming nurses’ leadership. Nurses can create “nursing journeys,” a profile where individuals discuss their career paths and offer reflections on lessons learned. This initiative provides leadership development for professional nurses in healthcare. Within the profile, the nurses discuss why they decided to get involved, the steps they took to get where they are today, the impact of the IOM report of Future of Nursing, reasons for involvement in the Campaign for Action, accomplishments, how nurses can improve health and healthcare in the US, and advice to other nurses and the next generation. This profile seeks to encourage other nurses to take leadership positions and directly impact healthcare transformation.   

Arizona Action Coalition

Like other states, the Arizona action coalition work with other stakeholders to transform nursing within the state. Arizona State’s action coalition initiatives focus on diversity and data. The alliance has created a comprehensive system for gathering medical workforce data from various roles and promoting awareness and use of the established health database resources (Campaign for Action, n.d-b). As a result, the alliance has improved resource awareness, broken down organizational and agency barriers throughout Arizona, connecting the state workforce institutes and resources around the country.

In regards to diversity initiative, the Arizona coalition, through its diversity council, has facilitated collaboration among the Black Nurses Association, National Association of Hispanic Nurses (NAHN), and the Philippine Nurses Association (Campaign for Action, n.d-b). Equally, the coalition has an established mentorship program for its nursing students.  

Barriers to Advancing IOM Report.

The most significant barrier to the IOM report is state legislation. The major areas of concern in states where the IOM proposals have faced opposition are challenged by medical APRN oversight requirements instead of nursing licensing boards (Altman et al., 2016). Such states also have a legal restrictions on clinical oversight, nurse-physician collaborations, and increased APRN scope.  

Furthermore, a section of physicians and medical groups has expressed opposition to the extension of the scope of practice of APRNs (Altman et al., 2016). Nurses, according to opinion shapers, are not seen as major policymakers or revenue creators. Therefore, nurses should be properly educated and should be included in decision-making with other healthcare professionals. 

Conclusion

The IOM report provided crucial recommendations for advancing nursing practice for the benefit of people and the government. With these recommendations, nurses would practice to the full level of their training and education. Equally, the recommendations would see improvement in nursing education standards to encourage innovation and effective nursing practices. Despite the barriers, the US and associated partners at the local and national levels continue to play significant roles in implementing the recommendations.
