For this assignment:
- Enter Sentinel City and click the map to locate the Health Department. Enter the Health Department and walk into the interview room. Once you enter the room, the Family Support Assessment Form will be available.
- Complete the Family Support Assessment Form by asking the client predetermined questions. Be sure to fill out the entire form. You can repeat the assessment, if needed. Click on Save and Review to open a PDF. Make any needed changes. Save the PDF to your computer. In the simulation, when the form is completed, click ‘Submit’.
- Then, outside of Sentinel City, develop a determinants of health assessment to address the potential concerns for this family.
- Which determinants of health do you think this family reflects and why?
- Select a health theory you have learned about that could explain this family’s actions and beliefs.
- What are some potential future issues that could impact this family based on your assessment?
- What interventions could be used to assist this family? What did you learn from completing this assessment in Sentinel City?
- Your assessment should be two to three pages in length (excluding cover page and reference page), and you should use at least two references. Your textbook can be one of the sources.
- Use current APA Style to style your paper and to cite your sources. Your sources should be integrated into the paragraphs. Use internal citations pointing to evidence in the literature and supporting your ideas. You will need to include a reference page listing those sources.
- Attach the PDF Family Support Assessment and your Word doc written assessment as attachments when submitting this assignment.
